High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink, in the mechanical room in the lobby hallway.

Intermediate - Lack of toilet tissue at each toilet. Observed : in the women's restroom, lack of toilet paper in the far stall.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed : lack of paper towels at the kitchen hand wash sink.

Intermediate - No probe thermometer provided to measure temperature of food products. Observed : no probe thermometer was found in the cabinets or drawers.

Intermediate - No soap provided at handwash sink. Observed : the soap dispenser at the hand wash sink in the kitchen would not dispense soap, it was dried in place soap. Dish soap was used to wash hands.

High Priority - Rodent activity present as evidenced by rodent droppings found. Approximately 8 droppings in storage cabinets under breakfast counter. Warning

Basic - Cove molding at floor/wall juncture broken/missing. Cove molding tiles missing near reach in cooler in back kitchen.

Basic - Dead roaches on premises. 1 dead roach on floor behind reach in cooler.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Shelves under Handwash sink and Three compartment sink soiled. Fan guard in reach in cooler in back kitchen. Drying racks drip tray is soiled.

Basic - Reach-in cooler shelves with rust that has pitted the surface. Reach in cooler in back kitchen the top shelf paint is chipping off exposing some rust.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Behind trash can in back kitchen and near Handwash sink.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: butter chips in a freezer food pan, on the counter in the kitchen, 62F. These had been out for service earlier, starting at 6 am.

High Priority - Toxic substance/chemical improperly stored. Observed: dog carpet shampoo stored over to go single serve in the dry storage room. The shampoo was moved to cleaning. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the reach-in freezer in the Market has spills on the bottom shelf.

Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed: lack of a chlorine test kit.

Intermediate - No probe thermometer provided to measure temperature of food products. Observed: lack of a probe thermometer.

Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed the front desk staff that handled the food service this morning lacks the Employee Reporting form. A blank copy was already available in an email.

Intermediate - No sanitizer of any kind available for warewashing. Only use single-service items to serve food to customers until sanitizer is available for warewashing. Observed: the kitchen lacked sanitizer. Chlorine bleach was brought from the laundry. Corrected On-Site
