Observed cooked rice held under time as public health control with no time marking. The operator voluntarily discarded the rice. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed the frozen drink machine nozzles with a black slime like substance. The operator cleaned the nozzles. Corrected On-Site

High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the dishmachines behind the outside bar at 25ppm. The operator has other dishmachines on site available for use. Corrective Action Taken

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at the hand sinks behind the bar.

Basic - Light shield damaged/in disrepair in the kitchen.

Basic - Standing water on bar top spill drain draining very slowly. Observed no drainage system for the spill ledge at the bar.

Basic - Waste line missing at soda gun holster behind the bar.
