Intermediate - Soap dispenser at handwash sink not working/unable to dispense soap to the right of the three compartment sink. Operator installed a new soap dispenser. Corrected On-Site

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw bacon in box stored above ready-to-eat lettuce in two door vertical reach-in cooler in prep room.

Basic - Cove molding at floor/wall juncture broken to the left of the three compartment sink.

High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee handle burger and burger bun with bare hands with the outside of the bun at 100F. Discussed requirements with employee and operator. Corrective Action Taken
