Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Hot water heater is super loud so operators are turning it on and off when needed. Spoke to operator ps about possibly getting a quite one. Corrective Action Taken

Basic - Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Operator has letters but has not placed on truck yet. Corrective Action Taken Repeat Violation

Basic - Hotel and Restaurant license decal not displayed on mobile food dispensing vehicle. Observe mobile food dispensing unit not having license number posted in front of unit with 2 inch numbers in contrast in colors Repeat Violation
