High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink missing automatic vacuum breaker on one side of splitter attachment.

Intermediate - No soap provided at handwash sink. No soap at hand wash sink at front of restaurant.

Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Hand wash sink by drive thru. Per manager, employee emptied pickle juice down sink and seeds are clogging it in pipes.

Observed 4 live flying insects in the following areas: 1 in mop sink area, 2 in dry storage area, and 1 in 3 compartment sink area.
