Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection observed ice buildup inside walk-in cooler. Repeat Violation

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Upon inspection observed grease buildup in bottom and behind cooking equipment. Repeat Violation

Upon inspection observed ceiling tiles and vents soiled with mold like substance and accumulation of dust in dish washer area. Upon inspection observed accumulation of food and grease on floor and back of equipment in the kitchen. Upon inspection observed accumulation of grease on the hood system in the kitchen. Repeat Violation

Basic - Equipment in poor repair. Upon inspection observed grills, cooler drawers stoves are all in poor repair in kitchen.

Basic - Floor tiles missing and/or in disrepair. Upon inspection observed missing tiles on kitchen floor in front of grills. Repeat Violation

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. At the hand washing sink in the dish room there were no paper towels. The manager refilled the paper towel dispenser. Corrected On-Site
