High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Rice in walk-in cooler at 50f for over 4 hours. See stop sale. Chicken in reach-in cooler at 46f for 2 hours. Operator applied ice bath under chicken to rapidly cool to 43f after 10 minutes. Corrective Action Taken

Basic - In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Scoop for rice in walk-in cooler has handle touching rice.

Basic - Utensils in poor condition. Observed operator scoop ice with ice scoop in back that is broken and lacks handle. Operator states she has ordered a new one. Corrective Action Taken

Basic - Ice buildup in walk-in freezer. Ice forming large pile on ground. Ice on closed boxes of fries. Per manager, repair company checked out freezer over the weekend.
