Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance in the kitchen area.

Basic - Clean equipment/utensils not stored at least 6 inches above the floor by the first drive thru window.

Basic - Floor soiled/has accumulation of debris in dining area and kitchen area.

Basic - Food stored on floor. Observed cooking oil on the kitchen floor by the broiler and posted mix on the floor underneath the register first drive thru window.

Basic - Ice buildup in walk-in freezer.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime in the employee hand sink in the front drive thru area on the fryers.

Basic - Reach-in freezer interior/shelves have accumulation of soil residues next to the fryers and the reach in cooler on the front line below the monitor.

High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. Observed employee washing there hands for 6 seconds.
