Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed several water stained ceiling tiles. Repeat Violation

Basic - Cutting board has cut marks and is no longer cleanable in the kitchen on prep tables.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the handles to several reach in coolers being soiled with food debris. Repeat Violation

Intermediate - Handwash sink not accessible for employee use at all times. Observed a cart being stored in front of handwashing sink near the dishwasher. Employee moved. Corrected On-Site Repeat Violation

Intermediate - No paper towels, soap or mechanical hand drying device provided at handwash sink. No soap provided at the handwashing sink near kitchen entrance. No paper towel provided at handwashing sink near dish machine.

Basic - Buildup of food debris/soil residue on equipment door handles. Observed the handles to the microwaves being soiled with food debris.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed water stained ceiling tiles in the dining area.

Basic - Current Hotel and Restaurant license not displayed. Observed expired license on display.

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed stacked wet containers on/near the cook line.

Basic - In-use tongs stored on equipment door handle between uses. Observed on oven doors.

Basic - Reach-in freezer interior have accumulation of soil residues.
