Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed self-service soda and ice dispenser machine with accumulation of black/green mold-like substance on soda nozzle and ice chute.

Basic - Ice scoop handle in contact with ice. Observed ice scoop handle in ice bin next to hand wash sink in kitchen area. Manager had employee remove ice scoop and place in proper storage area. Corrected On-Site

Basic - Single-service articles improperly stored. Observed coffee filters not covered for protection. Manager placed coffee filters under protective covering. Corrected On-Site

High Priority - Toxic substance/chemical improperly stored. Observed oven cleaner hanging on chemical storage above three compartment sink improperly stored.

Basic - Food stored on floor. Observed pickles in bucket stored on floor in walk in cooler. Observed napkins and fries cups in boxes stored on floor in back storage room.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile missing in dining room area and water damage to tiles.

Basic - Stored food not covered. Observed French fries in walk in freezer not properly closed for protection. Observed croissant not covered in back of kitchen.

High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed two cooks come from outside into kitchen put face mask on and hats and return to work without washing hands.

Intermediate - Handwash sink used for purposes other than handwashing. Observed hand wash sink in back storage area with a coffee cup in sink. Corrected On-Site

Basic - Ceiling tile missing. Observed ceiling tile missing in lobby and over the mop sink area, and water damage to ceiling tiles in the dining area.
