Basic - Floor soiled/has accumulation of debris. Observed syrup on the floor by the bag in a box rack next to the ice machine.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup on the fan guards in the walk in freezer.

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food.observed employees wearing bracelets and a watch, manager corrected and asked them to be removed. Corrected On-Site

Basic - Single-service articles not stored inverted or protected from contamination. Observed coffee and tea filters not in a protective container or inverted , manager inverted filters . Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed build up of ice on the floor of the freezer by the outside door, and on the condenser. Repeat Violation

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee put on gloves without first washing their hands.

Intermediate - Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed no hot water at handwashing sink next to microwave on the cook line, operator stated that the repair person was on the way to repair. Corrective Action Taken

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on walk in freezer near the door and on the condenser.

Basic - No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign in men's restroom.
