See stop sale. Pan of vinaigrette with cut tomatoes and onions with date mark of 3/25 on shelf inside walk-in cooler. Manager stated they cut the tomatoes themselves. Manager discarded the item during this inspection.

One employee handling food in kitchen without proof she has been informed of her health reporting responsibilities. Manager stated the employee was hired three weeks ago. Inspector provided manager a new copy of the employee health reporting agreement.
