Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile cracked in fryer area.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed bucket black mold like substance on gasket and frame of reach in cooler next to fryers.

Basic - Plumbing system in disrepair. Observed black mold like substance around pipe from previous leak under 3 compartment sink. Operator ids in the process of cleaning the pipes. Corrective Action Taken

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed black mold like substance on wall and door of walk in cooler.

Basic - Inaccurate/damaged temperature and/or pressure gauge(s) on dishmachine. Manager has called for repair.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed grease buildup on hood filters.

Basic - Floor soiled/has accumulation of debris. Underneath the equipment and shelves throughout the kitchen.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in the mop bucket.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet, where splitter was added. Repeat Violation

Basic - Hood ventilation system inadequate as evidenced by grease accumulation on walls/ceiling. Observed soiled hood vents, with grease running down side of hood interior, at fry station. Observed grease on side of fryer at fry station. Repeat Violation

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed three handles from cups in food at pizza make station. Handles removed from food. Corrected On-Site
