High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed scallops at 45F, chicken 46F, operator stated they had been in and out of cooler during lunch. Operator put ice on items to bring down temperature rapidly. Corrective Action Taken

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of ice machine with black mold like substance. Operator cleaned and sanitized areas. Corrected On-Site

Basic - Buildup of food debris/soil residue on equipment door handles. Observed reaching cooler door handles in back storage area next to reach-in freezers with excess soil and food particles.

Basic - Dead roaches on premises. Observed 1 dead roach near back exit door. Operator removed roach and cleaned the area. Corrected On-Site

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap at threshold of exterior exit door in back next to reach-in freezers.

Basic - Floor tiles missing and/or in disrepair. Observed floor tiles missing under reach-in freezer in back food storage area.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hood filters with excess grease and soil.

Basic - Ripped/worn tin foil used as shelf cover. Observed torn ripped foil on the bottom shelf of food preparation tables in kitchen.

Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed unsealed concrete floors in kitchen/ food preparation areas.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop not hung to dry. Operator hung mop to dry. Corrected On-Site

Basic - Working containers of food removed from original container not identified by common name. Observed multiple working containers of food in reach-in coolers in kitchen and in back reach-in cooler without labels.
