High Priority - Toxic substance/chemical improperly stored. Observed bio foam stored next to filter paper for oil cleaning. All chemicals from that shelf were moved below. Corrected On-Site
Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed paper towels were not available at the first hand wash sink. Towels were added. Corrected On-Site
Basic - Bulk milk dispenser with dispensing tube longer than one inch protruding from the chilled dispensing head. Observed the bulk dairy products on the front line were cut straight across. An example straw was cut and used to correctly cut the tube. Corrected On-Site
Basic - Bulk milk dispenser with dispensing tube not cut at an angle. Observed the front line dairy dispenser tubes not cut at an angle. These were then cut correctly. Corrected On-Site

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone on prep counter in kitchen next to salad station. Manager removed during inspection. Corrected On-Site
Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed build up of old food and debris on surface near grill station. Also observed build up of debris on cold holding equipment in kitchen area.
Basic - Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean. Observed on counter in front area next to ice cream machine wet soiled wiping cloth on counter. Manager removed during inspection.
