Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed an employee wearing a bracelet while preparing pizza at preparation area. Operator removed the bracelet. Corrected On-Site

Basic - In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed the utensil handle touching the mozzarella cheese at reach in cooler. Operator moved the utensil handle. Corrected On-Site

Basic - Accumulation of debris on exterior of warewashing machine.

Basic - In-use spatula stored in cracks between pieces of equipment. At reach in cooler preparation area. Manager removed the spatula and sent it to the three compartment sink to be cleaned and sanitized. Corrective Action Taken

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in freezer gasket soiled.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up in the dough freezer by the pizza preparation area reach in cooler.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed the black scoop of the mozzarella cheese used in the reach in cooler touching the food

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed the vacuum breaker missing at the hose section of the splitter Repeat Violation

Basic - Ice buildup in reach-in freezer at the bottom of the dough reach in freezer

Basic - Old labels stuck to food containers after cleaning. Observed old labels on the french fries and chicken nugget trays of the small reach in freezer next to the deep fat fryers . The old labels were removed Corrected On-Site

Basic - Wet mop not stored in a manner to allow the mop to dry. Repeat Violation
