Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed walk in cooler with rusted shelves. Operator placed order for new shelves. Corrective Action Taken

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling by the bar area in disrepair.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed fan covers in walk in cooler with dust.

Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.

High Priority - Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dishwasher machine not properly sanitizing due to no sanitizer attached to machine. Establishment has 3 compartment sink next to dish machine that uses quaternary sanitizer.

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw chicken over cooked chicken in the reach in cooler by entrance of establishment. Operator stored properly. Corrected On-Site

Basic - Accumulation of pink mold-like substance in the interior of the ice machine/bin. Observed ice machine closes to the door with pink mold like substance.

Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed mahi mahi inside reach in cooler thawing inside reduced oxygen packaging. Discussed with operator the correct procedure.
