High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed blue cheese at 48F at prep table across fryer station. As per operator less than 1 hour. Operator discarded and placed a new batch. Corrective Action Taken

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed vacuum breaker on the mop sink is missing its top cap and also missing a vacuum breaker at splitter added to hose Bibb at mop sink.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed soiled spray hose at Prep sink across from 3 compartment sink. Observed soiled with dust and grease the top of the hot holding unit for fries.
