Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like growth on the right corner of the ice bin.

Basic - Floor soiled/has accumulation of debris. Observed floor very sticky in front frozen machine.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime.Interior of fryer container and filters are soiled.

Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again.Pulled pork in reach in cooler.

Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present.Base of back door.

Basic - Ventilation system inadequate as evidenced by condensation on the air conditioning vents.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed peeling paint on the vent near 3 compartment sink, Sous chef notify engineering. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust accumulation on the ceiling tiles throughout the kitchen. Manager notify engineering department. Corrective Action Taken Repeat Violation

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed mold like growth on the wall around the caulking of the three compartment sink. Manager notify Engineering. Corrective Action Taken
