Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed yellow and red, green cutting board soiled stored on the counter on the cook line. Slicer blades used for the chopper has old food dried on the surface.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine white panel has pink discoloration on the surface.

Basic - Floor soiled/has accumulation of debris. Observed kitchen floor has food spilled on the floor throughout the back prep kitchen.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed stove and fryer has terrible grease accumulation and carbon build up left on the back panel located on cook line.
