Basic - Dead roaches on premises. In electrical room in back of restaurant approximately 1 dead roach on floor, discussed with operator who cleaned and sanitized area. Corrected On-Site

High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At steam table: French fries (114F - Hot Holding) As per operator, stored since 10 mins. Operator discarded. Corrected On-Site

Basic - Floor tiles missing and/or in disrepair. Observed missing and broken floor tile on front line.

Basic - Ice buildup in reach-in freezer.

High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed quaternary sanitizer at 0PPM. Operator corrected to 200PPM. Corrected On-Site
