Intermediate - Spray bottle containing toxic substance not labeled. Observed yellow chemical unlabeled bottle. Manager said it was peroxide cleaner. Then the manager took bottle and labeled it. Corrected On-Site

Basic - Equipment in poor repair. Door hinge detached from cooler on the cook line near hand washing sink. Observed cook line grill had no knobs to operate. Tools have to be used to turn on.
