High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed food handling employee plating lettuce and tomato for hamburger with bare hands. Observed hands were washed before bare hand food contact. After discussion, operator washed hands and put on gloves. Corrective Action Taken

Intermediate - Spray bottle containing toxic substance not labeled. Unlabeled spray bottle of cleaning chemical on shelf. Operator labeled and corrected storage of chemical during inspection. Corrected On-Site

Basic - Wet mop not stored in a manner to allow the mop to dry. Wet mop stored on floor of mop sink.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handle for flour. Operator removed scoop from flour during inspection. Corrected On-Site
