Basic - Floor area(s) covered with standing water. Standing water on floor in left side walk in cooler.

Basic - Current Hotel and Restaurant license not displayed: observed the establishment's license is current, however, they don't have a copy of it on hand and/or displayed.

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Raw Chicken nuggets stored over ready to eat brownies.

Basic - Floor area(s) covered with standing water. Standing water in walk in cooler. Repeat Violation

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup on bottom of condenser and on floor underneath condenser unit in walk in freezer.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Repeat Violation

Intermediate - Non-pitting surface rust on food-contact equipment. Racks in walk in freezer with rust present.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Walk in cooler has accumulation of dust/ mold like substances.

Basic - Cove molding at floor/wall juncture broken/missing. Multiple areas in back of kitchen and walk in coolers with missing or broken cove molding tiles.

Basic - Floor area(s) covered with standing water. Upon inspection, standing water observed in multiple areas of establishment.

Basic - Floor soiled/has accumulation of debris. Back right corner of cooler has debris present, and floor in front of drink machines is soiled.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up on gasket off reach in freezer across from fryers. Ice buildup in walk in freezer in back walk, under condenser. Ice buildup in reach in cooler near hot holding/prep line. Repeat Violation

Basic - Objectionable odors in chicken walk in cooler.

Basic - Water dripping on floor surface in back of house.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Basic - Water draining onto floor surface. Upon inspection observed multiple ceiling areas in kitchen with water dripping on floor. Did not observe any cross contamination.

Basic - Water leaking from pipe and/or faucet/handle. Mop sink faucet leaking.
