High Priority - Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed the dishwasher eat a banana then grab clean dishes without washing his hands. Operator had the employee wash their hands and rehash the dishes. Corrective Action Taken

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on the server line: Cut fruit (57F - Cold Holding); Pico de galo (57F - Cold Holding). Operator stated they had been out approximately 3hrs. Operator placed them in a reach in cooler.
