Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign at sink in dish area.

Basic - Door to outside going towards walk in cooler soiled with accumulated grease, food debris, and/or dust.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee beverage on food preparation table. Observed employee remove beverage. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed grease buildup fry vats exterior. Observed grease buildup on exterior of pots. Observed soda machine exterior with mold like substance.

Basic - Nonfood-contact surfaces not designed and constructed to allow easy cleaning and maintenance. Observed particle board wood surface on wall in ware washing area used to store clean equipment. Observed cardboard lining equipment shelves in ware washing area. Observed shelves in ware washing area for clean equipment rust pitted.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed drink station soiled with mold like substance.

Basic - Cleaned and sanitized equipment or utensils not properly stored. Observed clean equipment stored on shelf with cardboard lining.

Basic - Interior and exterior of oven/microwave has accumulation of black substance/grease/food debris.
