Observed no employee health policy or evidence of employee health training present in the facility. CA: PIC must provide documentation or otherwise satisfactorily demonstrate that all food employees and conditional employees are informed of their responsibilities to report to management information about their health and activities as it relates to diseases that are transmissible through food. COS: All employees must receive employee health training that is verifiable through signed documentation of an employee health policy (either physical copy or digitally). PIC provided with blank example copy.

Observed old inspection report displayed publicly in the lobby and no inspection report displayed in view for drive thru customers. CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. Establishments with drive thru windows must have at least the top portion of the current inspection report posted allowing customers to easily read the score, date of inspection and establishment information. COS: PIC given two printed copies of the inspection report and will display upon receipt.
