EHS observed multiple bulk food storage containers (MSG, flour, fish fry, sugar) with no label of contents.
511-6-1.04(4)(d) - Food Storage Containers Identified with Common Name of Food (C) (d) Food Storage Containers, Identified with Common Name of Food. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.
CA: EHS advised PIC that containers must be labeled with common name of the food contents.

EHS observed a package of expired Coleslaw in the walk-in cooler; packaged exp. 01/10/26.

Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. P
CA: EHS advised PIC to ensure items are checked regularly for the used by date, expired items must be discarded.
12A - contamination prevented during food preparation, storage, display

EHS observed: multiple food containers/boxes in the walk-in freezer stored directly on the floor; box of plastic bag for take-out containers stored directly on the floor

- Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food:
(i) In a clean, dry location;
(ii) Where it is not exposed to splash, dust, or other contamination; and
(iii) At least 6 inches (15 cm) above the floor.
- Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts.
CA: EHS advised the PIC that food packages must be stored above the floor; PIC moved items.
14C - single-use/single-service articles: properly stored, used

EHS observed multiple single-use containers being re-used, either to store utensils or as scoops in products.

The bulk milk container dispensing tube shall be cut on the diagonal leaving no more than one inch protruding from the chilled dispensing head.
CA: EHS advised the PIC that single-use items can not be re-used; PIC corrected observed items.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
EHS observed no designated for employee personal items, numerous personal food items were observed stored near food product of the establishment. Employee devices were observed in working areas near single service articles.

Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.

Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.
CA: EHS advised the PIC that a designated area for employee food items must be established to prevent cross contamination, and personal devices should not be in working areas near single-service articles.
