EHS observed the water temperatures in rooms 101, 102, 105, and 254 to be above 120F; ranging from 135-136.

511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

CA: EHS advised the PIC that the hot water temperature in all guests rooms should not exceed 120F and be 100F at minimum. The PIC stated that he would have their contracted plumbing ajust the thermostate because they prefer not to

Corrective action: Spoke with PIC to adjust water temperatures in these room to ensure thye remain below 120F

EHS observed debris and stain in the following rooms:

Room 101- Hair on mattress sheets. Room 102- Debris on mattress sheets. Room 204- Debris on sheets.

CA- PIC needs to make sure that housekeeping is changing each mattress with clean sheets, comforter, and mattress pads.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

EHS observed a lizard underneath the bedding. In room 103 EHS observed a dead bug on the bathroom floor near the bathroom entrance. In room 139 there was a dead roach in the pull out drawer. In room 203 there was a dead bug behind the toilet.

Corrective action- EHS informed PIC to make sure housekeeping is cleaning properly and making sure that the rooms are clear from dead insects.

511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program

EHS asked for the gas/fire inspection report. The property did not have the previous inspection on file at the property.

Corrective action- EHS informed the PIC to make sure that when they are out of the office to make sure that someone in the office has access to the gas/fire inspection report.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

EHS observed toilets dirty in rooms 103, 204, 211, 254. Also EHS observed mold like substance on the toilet seat in room 142.

Corrective action- EHS inform the PIC to make sure that housekeeping is properly cleaning the bathrooms fully.

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists
