EHS observed a lizard underneath the bedding. In room 103 EHS observed a dead bug on the bathroom floor near the bathroom entrance. In room 139 there was a dead roach in the pull out drawer. In room 203 there was a dead bug behind the toilet.

Corrective action- EHS informed PIC to make sure housekeeping is cleaning properly and making sure that the rooms are clear from dead insects.

511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Corrective action- EHS informed the PIC to make sure that when they are out of the office to make sure that someone in the office has access to the gas/fire inspection report.

While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services.
