Observed hot water over 120F in majority of the rooms; water temperatures ranged from 115F-137F. 511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. EHS informed the manager of the need to adjust the hot so it does nor exceed 120F

Observed facility's need to clean AC filters and units in majority of the rooms. All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. EHS informed the manager that the AC unit must be cleaned routinely

Observed hair and/or debri and stains on mattress pad covers in rooms 117,407,411. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. EHS informed the manager of the need to change out the bedding in the noted rooms

Observed hair on the floor in rooms 117 and 407. oilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists.

EHS informed the manager of the need for staff to thoroughly clean the rooms

Observed damaged flooring, flooring underneath AC units(unsealed), and coving in majority of the rooms. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored. EHS informed the manager of the need to have the flooring and coving repaired in rooms where it is damaged

Observed missing wire plate on wall behind the TV in room 407. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored. EHS informed the manager of the need to install covering as soon as possible

Observed ice machine on the 4th floor in need of cleaning; pink/black substance is covering the chute. EHS informed the manager of the need to clean the ice machine on the 4th floor. *Ice machine of the first floor is out of order

EHS observed the following water temperatures that exceeded 120: Room 507-water-127 Room 506-water-130 Room 500-water-140 Room 502-water-139

CA-PIC needs to inform maintenance that the water heater temperature needs to be lowered. Water temperatures in the guest rooms need to e between 100 -120 degrees.

EHS requested for the gas-fire inspection report. PIC presented a out of date report from 2/9/2024. He stated that the plumber is coming to service them in a few weeks.

CA-PIC needs make sure that the gas-fire inspection up to date.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

EHS observed the following the following each room:

Room 507-yellow stain on mattress pad and black spots on the mattress pad. Room 506-hair and stains on the mattress pad. Black spots on the sheets. Room 500-hair and yellow stains on the mattress pad. Room 502-stained comforter and pillowcases are stained. Yellow stain on mattress pad.

CA-PIC needs to make sure that housekeeping is cleaning and changing all bedding items. If the bedding items are damaged then the items must be properly discarded and replaced with new ones.

EHS observed water exceeding 120 F in all rooms observed.

Hot water in all guest rooms shall not exceed 120F.

EHS recommended to PIC to have maintenance turn down all accessible boilers and to have thermometers on hand to measure in-room temperatures

EHS observed hot water exceeding 120 degrees Fahrenheit in rooms 407, 405, 504, 512, and 603. Violation: hot water to all guest rooms shall not exceed 120 degrees Fahrenheit where possible. CA: adjust water heater to not exceed 120

EHS observed dust accumulation on air filters rooms 407, 405, 504, 503, 512, 514; and broken air filter room 403. Violation: all furniture, waste receptacles, draperies, carpets, appliances, and accessories shall be maintained in good repair, clean and free of vermin. CA: clean air filters at frequency to prevent accumulation of dust
