EHS Observed: Hot water temperature exceeded 120F in all rooms - temps ranged from 125-130F.

511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

CA: Informed PIC that observed water temps were too high and should range between 100F and 120F

EHS Observed: PIC unable to provide gas-fired equipment inspection report for view at time of inspection. PIC indicated that the last inspection was performed in November of 2024 but he is unsure where the report was stored.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

CA: Informed PIC that the gas-fired equipment inspection is to be performed annually. Advised PIC that the inspection report should be stored locally for view at time of inspection

EHS Observed:

• Dirty A/C unit air filter in all rooms
• Room 150: Stained and damaged sofa; interior of microwave rusting
• Room 148: Damaged furniture upholstery

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

CA: Advised PIC to increase cleaning frequency of A/C air filters. Advised that stained and damaged furniture should either be repaired of replaced. Advised that damaged appliances should be repaired or replaced

EHS Observed: Stains, hairs, and/or debris on the linen of all rooms.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: Advised PIC that linen should be cleaned and replaced, and that items from which stains can't be removed should be discarded and replaced

Observed no gas-fire equipment record available at time of inspection. PIC mentioned that they had just replaced the water heater 6 months ago, but could not provided documentation.

-150: Microwave with food debris, ice build up in fridge, air conditioning filter with debris accumulation. -148: Ironing board damaged, damaged desk lamp shade, desk and arm chair damaged, microwave missing, air conditioning filter with debris accumulation. -147: air conditioning filter with debris accumulation. -149: Ice build-up in fridge, damaged and stained couch.

EHS observed damaged coverlets/cigarette burns in all inspected rooms.

EHS observed stains on coverlets/comforters in all inspected rooms.

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin.

EHS advised PIC to replace burned coverlets, increase cleaning frequency

EHS observed live roaches and roach droppings in dresser drawers in rooms 147, 145, 145, 260, 255.

Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program.

EHS informed PIC of need to increase cleaning frequency and to bring up the issue with pest management company owner employs

EHS observed dust accumulation in all rooms inspected (147, 145, 149, 260, 255), along with damaged filters in room 145;

Stains on curtains in rooms 145, 147;

Debris in drawers of all observed rooms, spills in drawers in room 260;

Damaged cabinetry in rooms 145, 260;

EHS observed damage coverlets in all rooms observed, including cigarette burns. Observed hair accumulation in bedding of room 260. Towels in room 145 were damaged and stained.
