EHS observed the following water temperatures in the corresponding room to be at/above 120F or below 100F:

511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

CA: EHS advised the PIC that hot water in all guest rooms shall not exceed 120F, and to ensure that maintenance adjusts the hot water source. Also, advised that maintenance investigate water fluctuation observed in room 302

EHS observed damaged and/or dusty A/C unit air filters in the following rooms:206, 216, 227, 302, and 312.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

CA: EHS advised the PIC that the damaged a/c air filters should be replaced with new ones, and to ensure that housekeepers are routinely cleaning the filters

EHS observed the ice machine on the 2nd floor of the facility to be out of order. The PIC stated that the unit has been down for a couple of weeks, and was unable to provide a work order for repairs when requested by EHS, stating that due to recent changes they were unsure when the machine would be repaired.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

CA: EHS informed the PIC that the ice machine should either be repaired, replaced, or removed from the property

EHS observed the incorrect TA inspection report to be on display for public viewing. The posted inspection report was dated April 11, 2024 with a score of 96; however, the most recent inspection was performed on July 03, 2025 and resulted in a grade of 91.

511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away.

CA: EHS advised the PIC that the most recent TA inspection report must be displayed for public view, and to ensure today's inspection is posted

EHS observed the following water temperatures:

CA-PIC needs to contact maintenance to adjust the hot water heater.

Room 418-hair on mattress pad Room 414-hair and stain on mattress pad. Room 309-stained mattress pad.

CA-PIC needs to make sure that housekeeping is cleaning all rooms with the clean bedding items. Any stained or damage items must be properly discarded and replaced.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

EHS observed the following rooms with exceeded water temperatures: Room 505-water temperature-138 Room 507-water-126 Room 420-water-128 Room 208-water-139 Room 209-water-136 Room 211-water-135

CA-PIC needs to contact maintenance to adjust the water heater between 100-120 degrees.

EHS requested for gas-fire inspection. The last inspection was January 2023. PIC did state that she contacted the company and they were coming out this week.

CA-PIC needs to make sure that the gas-fire inspection is done annually. 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

EHS observed the following rooms with dusty air filters: Rooms: 506,514, 424, and 420.

CA-PIC needs to make sure that housekeeping is cleaning all air filters routinely to reduce dust build up in filters.

EHS observed the following in each room:

Room 514-black stain on sheets. Room 424-pink stain on mattress pad and pillow case. Room 420-stain on mattress pad and mattress sheets.

CA-PIC needs to make sure that all bedding items are clean and free from debris and stains. If the bedding items are damaged then they will need to be properly discarded and replaced with new ones.

EHS observed the personal items mixed in with items that is used for the hotel. EHS observed a bag stored beside bedding items on the table. EHS observed a water bottle in the storage room on right beside were clean sheets are stored.

CA-PIC needs to create a designated area for employees to place their personal items. Personal items should not be stored near items that the hotel uses.

511-6-2-.14(7) - Sleeping Quarters Separation Sleeping quarters must be separated by a wall from the food preparation, food storage, and food service areas of a bed and breakfast inn

EHS observed the following in each room:

Room 214- Dusty air filters Room 223-Dusty air filters Room 224- dusty air filters Room 228- Dusty air filters Room 394- dusty air filters Room 307-dusty air filters and damaged vents on the ac unit. Room 310-dusty air filters Room 311-dusty air filters Room 312-Dust air filters.

Corrective action- PIC needs to ensure that housekeeping are routinely cleaning the air filters in each room.
