EHS requested pest control records from PIC. PIC informed EHS that they do not have records to provide during the TA inspection.

CA- PIC needs to make sure they have documentation of pest control inspections. This documentation needs to be located on the property and easily accessible to the PIC and/or employees.

511-6-2-.13(2) - Insect and Rodent Control-Records A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator
