Observed cheese, sauce, and lettuce above 41F in prep top cooler. COS items were disposed of. CA repair prep. top cooler.

Observed chicken nuggets being hot held at a temperature below 135F. COS items were disposed of. CA repair heat lamps.
11D - thermometers provided and accurate
Regulation: 511-6-1.05(2)(x) - tmd, located in refrigerators/ hot holding (pf, c)
Observed that facility was missing ambient thermometer in prep top cooler. CA provided thermometer.
15C - nonfood-contact surfaces clean

Observed food debris and grease build up on and behind equipment through out kitchen area. CA Follow all eco lab instructions and clean areas.
