Person in charge (PIC) not ensuring or monitoring that employees are using proper reheating methods to ensure previously held at 41F TCS food is reheated to 165F or same day prep ready to eat food is initially heated to 135F and held at 135F. Code: PIC's duty is to make sure employees are properly cooking, cold/hot holding, and reheating for hot holding time/temperature control for safety food...through daily oversight of the employees' routine monitoring of the cooking, holding, and reheating for hot holding temperatures using appropriate temperature measuring devices properly scaled and calibrated.

Observed order of chicken frozen onsite above fries in freezer. Code requires the PIC to ensure employees are visibly observing and verifying delivered foods as they are received to determine that they are from approved sources and are placed into appropriate storage locations, as required by this Chapter.

Observed raw chicken in covered plastic containers at 43F at both cooling temperature checks at 3:23 pm and 4:13 pm. CA: Uncover items cooling from room temperature preparation, use metal containers that facilitate cooling instead of plastic containers that restrict cooling, separate into thinner portions, and/or any other effective means of cooling.
11C - approved thawing methods used

Observed thawed fish in drawer cooler in plastic packaging. COS: removed from plastic packaging. Code: Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment.

Observed a couple of house flies in kitchen. CA: Continue to work on pest control measures.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Using plate as scoop as evidenced by the plate left in the rice. CA: use utensil with a handle and store according to code. Code: Food preparation and dispensing utensils shall be stored: in the food with their handles above the top of the food and the container.

Bathroom walls are unsealed brick. CA: Contact health department for approval of material prior to properly sealing walls.
