Observed rust and/or food residue in the microwave in rooms 32, 33, 34, and 35.

CA: Charlton EH instructed PIC to clean any microwaves of food reside and to replace any microwaves that have rusted interiors.

Observed rusted refrigerator shelving in room 33.

CA: Charlton EH instructed PIC to seal or replace any rusted shelving in refrigerators.

Observed stains on the chair cushion in rooms 32 and 33.

CA: Charlton EH instructed PIC to clean or replace

Observed damaged wall section between bathtub outlet and wall.

CA: Charlton EH instructed the PIC to seal the damaged section

Observed daylight through walls of laundry facility.

CA: Charlton EH instructed PIC to seal up the walls from the outside

No records of annual gas fired equipment inspection. PIC stated they had it done but could not provide evidence.

CA: Charlton EJ told PIC to have gas fired equipment inspected immediately and to have the inspection on file

Observed severely stained pillows, linens with hairs, no pillow protectors for any of the pillows, and some mattresses missing mattress protectors.

CA: Charlton EH told PIC that they are required to replace when severely stained and that they must have pillow and mattress protectors

Trailer spaces for RV's are not clearly marked. A broken chair was outside of a room to be rented being used as outdoor seating.

CA: Charlton EH told PIC that trailer spaces must be clearly marked per the TA rule and that all furniture must be in good repair

Bathtubs have peeling paint due to repairs made and then painted over. Bathtub drains are missing covers or not attached in bathrooms as well as the faucets not flush with the wall. Vent cover in bathroom of room 24 about to fall due to being attached by one screw. Damaged soap holder room 26. Door frame damaged, exposing interior of building.

CA: Charlton EH told PIC to cease renting room 24 due to vent cover until it is secured to ceiling and recommended replacing the bathtubs

Observed bathroom vents not working or missing covers.

CA: Charlton EH instructed PIC to have them repaired/replaced

PIC was unable to show records documenting pest control from contracted company they use.

CA: Charlton EH reminded PIC that they are to keep pest control records for eighteen (18) months

Observed damaged walls in laundry.

CA: Charlton EH instructed PIC to have the holes in the walls patched/sealed to prevent pest activity

Rodent feces observed in the laundry room on the floor, shelves, and on clean linens. One room had animal feces under a chair cushion.

CA: Told PIC to have pest control target for rodents, seal up gaps in the laundry room, and to reclean the soiled linens. Talked to PIC about keeping A/C to keep rooms a different temperature than outside to discourage lizards/bugs from entering rooms

No records of annual gas fired equipment inspection.

POC: Told PIC to have gas fired equipment inspected annually and documented

Observed pillows/pillowcases with stains on them and debris between sheets.

CA: PIC told to replace stained pillows or pillowcases, any linen with stains and to ensure clean linens are placed on beds

Damaged tub and sink drains (missing covers or not attached) in bathrooms. Vent cover in room 31 about to fall due to being attached by one screw.

CA: PIC to have damage repaired/covers replaced. Vent cover to be replaced and attached correctly

Laundry/linen storage area has gaps leading to the outside, not screened.

POC: PIC told to seal up gaps and install more screens as any daylight that can be seen is an entry way for pests

Observed debris (trash, lint, dead bugs) under the beds in rooms inspected. Broken mirror in one room.

CA: Told PIC to have housekeeping clean under beds again and to check behind them and to replace mirror

No records of annual gas fired equipment inspections. Facility has two gas water heaters. POC: Told PIC to have gas water heaters inspected annually and documented

Observed trash and debris under the beds in rooms inspected. Rooms 32 and 19 had a couple of dead bugs (beetles) on the carpet; one of these rooms had just been cleaned. CA: Told PIC to have housekeeping clean under beds and check their vacuums
