Observed raw chicken, raw shell eggs, and cooked carnitas cold holding above 41F in walk in cooler. COS: PIC discarded items.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed black build up on ice baffle of ice machine. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed excessive fly activity in the facility. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(g) - mechanical ventilation, sufficient capacity to prevent buildup of heat, steam, smoke, fumes & odors (c)
Observed insufficient capacity of mechanical ventilation in physical facilities to prevent excessive heat and contamination of food items. CA: Mechanical Ventilation. If necessary to keep rooms free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke, and fumes, mechanical ventilation of sufficient capacity shall be provided.

Observed employee drink and items stored above clean dishes in the ware washing area. COS: PIC stored items in employee designated areas.
