Observed TCS food items (shredded cheddar, sliced cheese, chicken wings, etc.) cold-holding in the prep-top cooler above 41 F. (COS- PIC disposed food items in the trash and plugged in prep-top cooler.) RCA: Ensure all cold TCS food items are held at 41 F or below at all times.
1-2B - certified food protection manager

Observed no CFSM certificate posted in public view. RCA: Ensure the certificate is posted in public view at all times.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(r) - food storage, prohibited areas (c)
Observed a box of frozen patties stored under condensation lines in the walk-in cooler. Box is now covered in ice. RCA: Ensure all food service items are stored away from leaking water lines, including condensation lines at all times.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed two in-use scoops stored in water at 77 F. (COS- PIC discarded water, placed utensils on clean deli paper.) RCA: Ensure in-use utensils are stored in still water of at least 135 F, stored in dipping wells, or leave out and clean every 4 hours.

Observed cup stored in the box of dehydrated onions. RCA: Ensure the scoop stored in non-TCS food items are stored with the handles sticking up.

Observed boxes of single use cups and food containers stored on the floor in the storage room. RCA: Ensure all single use items are stored at least 6 inches off the floor at all times.
