Facility PIC could not provide an annual gas fired equipment inspection form. RCA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed an excessive build up of a black debris on the baffle of the ice machines on the 1st and 2nd floor.

RCA: Ice machines must be maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Ice machines must be clean at a frequency that prevents the build up of any debris

Observed multiple spray bottles of chemicals to not be labeled. RCA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed the hot water in rooms 221, 225, 309, and 320 to exceed 120F. See temperature log in the comments for individual room hot water temperatures.

RCA: In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

Observed yellow staining on the comforter in room 315, and red staining on the comforter in room 320. Observed yellow staining on the mattress pad in rooms 305 and 320. Observed yellow staining on the mattress cover in room 305.

RCA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed brown like stains on drapes in rooms 224 and 206 at window.

RCA: 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean,

Observed the following rooms found various brown staining on mattresses, comforters, mattress pads, and sheets:300,309,314,326,224,219, & 206. Observed the following rooms with brown stains on pillowcases:309,219, & 206.

RCA:11-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase

Observed the following rooms with soft spots on the floors near A/C units by the window: 300 and 206. Observed in room 206 water damage to the ceiling and chipped paint above A/C unit near window.

RCA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed heavy black like substance build up on the baffle of the ice machine on the 1st,2nd and 3rd floors.

RCA: All ice machines shall be shut down and serviced until the ice machine is cleaned and in good repair. The cleaning frequency of the ice machine shall be increased to prevent build up
