Observed multiple handwashing sinks without hand drying methods available.
COS CA: PIC stocked paper towels at stations. Each handwashing sink or group of adjacent handwashing sinks shall be provided with an approved hand drying provision.

Observed reduced oxygen fish completely thawed in packaging despite bearing a label indicating that it is to be kept frozen and removed from the reduced oxygen environment prior to its thawing and/or immediately upon completing its thawing.
COS CA: PIC discarded salmon. ROP fish must be completely removed from package when thawing. Reduced oxygen fish that bears a label indicating that it is to be kept frozen must be removed from the reduced oxygen environment prior to its thawing and/or immediately upon completing its thawing.

Observed dishwasher in bar area without chlorine testing strips available. (3-compartment kitchen dishwasher is a high heat sanitizer)
RCA: . A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided.
