Observed an accumulation of food residue and other debris in the bottom of a reach in freezer next to grill area, under the toaster, and on the top of the fryer.
RCA: Non food-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed an accumulation of grease under fryers on floor and dirty water puddles on floor near dry storage area.
RCA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Observed raw bacon stored above cooked, RTE burritos and pancakes in the walk-in cooler. COS: PIC rearranged food items based on cook temps and will ensure it remains this way.

Observed hand sink used as a dump sink near fry station. CA: PIC will monitor and ensure that all hand sinks are used for handwashing ONLY 2. A handwashing facility may not be used for purposes other than handwashing. Pf

Observed mop stored in mop water in main kitchen area not in active use. COS/CA: PIC removed mop to air dry and dumped water. PIC will ensure proper storage and use of mops.

Observed an employee wearing gloves. When said employee dropped a knife on the floor, they grabbed the knife and touched the floor in the process. Employee began to reach for sliced tomatoes to continue prepping before changing gloves and washing hands. CA/COS: Employee was stopped before they touched food items, but was told to wash their hands and dawn gloves appropriately.

Observed TCS food items (cheese) being held using TPHC with a time exceeding the 4 hour limit, and also the wrong date. Food items were dated for discard 5/29/23 at 5:17am. CA/COS: PIC discussed the time and dates were wrong but the food items were just pulled within the last 30 min. Temp was at 48F and was still in "SAFE ZONE". PIC had employee relabel food items. If time without temperature control is used as the public health control, food shall be discarded when this time has been reached and labeled correctly.
