Observed that updated gas fire equipment form not on file. Last gas fire equipment form was from 2023.
RCA: PIC was informed that gas fire equipment form should be completed annually. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority

Observed that multiple rooms had stains or cigarette burns on the bedding. See comments.
RCA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed Air conditioning filters in hallway floors of 2nd & 3rd floors with heavy accumulation of dust. Observed air conditioning filters in room 302 with heavy accumulation of dust.
RCA: 511-6-2-.14(3) - Room Ventilation Ventilation shall be provided for all rooms. Where ventilation is provided by means of windows, they shall open directly to the outside air and the openable window area of each room shall be not less than 1/20 of the floor area served. Where ventilation is provided by other means, it shall comply with the requirements of the current International Mechanical Code with Georgia Amendments or future subsequent revisions adopted thereafter to provide comfortable living conditions, remove objectionable odor and fumes, and prevent excessive condensation. Ventilation systems shall comply with applicable State and local fire prevention requirements and building codes

Observed Lobby, 2nd & 3rd floors ice machines with heavy build up on baffles. Observed lobby ice baffles with black build up on corner. Observed 2nd floor ice baffle with heavy build-up of a pink slime-like substance. Observed 3rd floor with heavy black build up on ice baffle.
RCA: 511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

Observed hair and staining on mattress protectors and sheets in all rooms observed. CA/COS: PIC will have housekeepers change out sheets and protectors. PIC will ensure housekeepers remove all hair and other debris before making beds. Housekeepers will treat the stains, and if they cannot be treated they will be discarded and replaced
