Observed salsas in the walk-in cooler; sliced tomatoes an fried chilies in the prep-top cooler and salsa in the bar cooler without a cover. (COS- PIC covered food items with a lid.) RCA: Ensure all food items are covered unless during cooling.

Observed two separate pans of chicken and beef cooling on the counter without a lid covering it. When cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination.
CA: PIC covered both meats with a lid to prevent any contamination to occur while both items were cooling.

Observed raw chicken stored in between raw beef and raw fish on the same shelf in the walk-in cooler. Observed Raw shelled eggs stored on bottom shelf next to chicken in the walk-in cooler.
COS/CA: PIC placed meats and eggs in correct separation order (chicken on the bottom, beef above chicken and below seafood, eggs above all). Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from: Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, and cooked ready-to-eat food.

Observed dish machine in the kitchen to not be pulling any sanitizer.
RCA: PIC will utilize 3-compartment sink until Ecolab can service dish machine.

Observed an employee phone sitting on a prep table next to plates in the kitchen area.
COS/CA: PIC had the phone removed and placed in a designated area.
