Observed organic growth on the sliced provolone cheese, hot peppers, and tomato slice in prep top cooler in kitchen. COS: PIC discarded food items.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(a) - handwashing cleanser, availability (pf)
Observed hand soap dispenser not working in men's restroom. CA: Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.

Observed a cart wheel in back handwashing sink. COS: PIC removed wheel and cleaned hand sink. A handwashing facility may not be used for purposes other than handwashing.

Observed dried debris on food contact surface of milk shake mixers. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
