Observed TCS items in reach-in cooler to be cold holding above 41 degrees (ham, potato salad). See temperature log. COS: PIC discarded all TCS items in reach-in that were held for more than 24 hours. Other items prepped this morning were moved into the walk-in cooler or placed in an ice bath to rapidly cool before being placed in walk-in. TCS foods shall not be held in reach-in until maintenance services equipment.

Observed chemical sanitizer in mechanical dishwasher not pulling adequate amount of sanitizer. Observed sanitizer for 3-compartment sink not pulling any sanitizer. CA: PIC refilled the sanitizer for that adequate amounts are supplied to dishwasher and 3-compartment sink.

Observed multiple bottles of bleach stored above clean dishes. COS: PIC removed all bottles of bleach to the floor away from the clean dishes.

Observed white mold-like debris on racks inside of walk-in cooler. Observed dust build-up on fans inside of walk-in cooler. Observed mildew-like substance on gaskets of multiple reach-in units.
RCA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
