Observed debris and/or stains on the bedding in rooms 10, 9, 8, and 6.

Observed hot water in rooms 3, 10, 20 to exceed the maximum 120F. RCA: 511-6-2-.07(2) - Water Supply and Sanitizing Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

Observed multi-use dishes and utensils in room 25 with no dishwasher or three-compartment sink for sanitation.

RCA: Water glasses, ice buckets and other multi-use utensils provided for tourist’s use in a guestroom or dwelling unit shall be washed, rinsed, and sanitized, in a 3-compartment, warewashing sink dedicated for that purpose only. They shall be thoroughly cleaned in the first compartment in a warm soapy solution; rinsed clean of soap and debris in warm clean water in the second compartment; and sanitized as specified in subsection (5) (d) 1, 2, 3, 4, and 5 using chemical sanitizers or sanitized by hot water as specified in subsection (5) (e), after each occupancy and as needed during occupancy. Where approved sanitizing methods are not provided, single service and single use articles, such as paper or plastic cups shall be made available in lieu of glasses and shall be discarded after one use. In lieu of sanitizing ice buckets, a sanitary, food grade, plastic bag, large enough to line the bucket and overlap the top edge may be provided for use by a single occupant

Observed staining on pillows, mattress, mattress pad, and/or sheets in rooms 9,10, 18, and 25. Observed hole in sheet in room 25.

RCA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed central ice machine with build-up on the baffle.

RCA: Ice machine shall be cleaned and sanitized at a frequency to prevent build-up

Observed damage outlet cover in bathroom of room 10. Damage caused a hole in the outlet cover that could allow water to come in contact with electrical lines.

RCA: Replace outlet cover

Observed hot water temperatures in rooms 9, 18, and 22 (132, 130, and 123, respectively) above 120 F. Advised PIC to ensure water temperature is not exceeding 120 F in all rooms

Observed stains on mattress covers of rooms 9, 18, 23, and 25. Also observed live insect made in bed of room 18. Advised PIC to ensure all beddings are clean, in good repair, and free of vermin

Observed green build-up on internal baffle of ice machine. Advised PIC to ensure ice machine is clean and free of debris at all times
