Observed multiple flies and gnats throughout the facility. Observed a dead roach with ants around it in the employee bathroom next to the toilet. RCA: Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Observed handwashing sink near three compartment sink in main kitchen to be leaking heavily when water is on. RCA: Facility shall repair sink such that it does when sink its turned on the water drains without leaking from piping. Food Service Rule: A plumbing system shall be repaired according to law; P and maintained in good repair.

Observed grease lining the back and sides of fryer. Observed quesadilla press to be heavily soiled. RCA: Facility shall clean around the exterior of equipment to ensure that there is no build up. Food Service Rule: The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
