COS CA: PIC discarded all thawed fish (salmon) that was still in ROP packaging. Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment.

RCA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

COS CA: PIC had employees remove foods and reheat. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above.

Observed mildew like substance built up on back and sides of inside of ice machine. Observed Ice scoop handle in contact with food, and ice scoop handle in contact with ice. Food debris was observed in rolling pins in metal drawer. RCA: Utensils shall be clean to sight and touch. Ice machine shall be cleaned regularly. Ice scoops and handles shall have handles to prevent contamination.
