Observed gaskets/seals on cold holding unit #3 (pass thru-reach in equipment) in poor repair and ambient Temperature of 65. Observed cold holding unit door falls down off the hinges when opened from the left side of the unit. This results in the following TCS items being stored above cold holding minimum of 41F: American Cheese sliced (62F), Sliced Tomatoes (59F), and Shredded Lettuce (65F).

Observed build-up on the baffle of the ice machine. Observed build-up on the ice machine's next to self-serve soda machines.
RCA: PIC shall increase the cleaning frequency of the ice machine so that it is clean to sight and touch. There shall be no build up on the machine. Current ice shall be melted and machine washed, rinsed and sanitized. Equipment, food-contact surfaces and utensils shall be clean to sight and touch.

Observed multiple flies throughout the facility.
RCA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: Routinely inspecting incoming shipments of food and supplies; Routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; and Eliminating harborage conditions.
