Observed slime and mildew-like substance on baffle of ice maker machine.
RCA: Equipment, food-contact surfaces, and utensils shall be clean to sight and touch.
18 - insects, rodents, and animals not present

Observed facility outdoor walk-in cooler had one door with a damaged lower weather strip allowing entrance to outdoor pests and outside contamination. Observed a large hole in wall surrounding where piping was entering.
RCA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by Filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed single-use items (cups, lids, to go containers) in plastic sleeves in boxes stored in outdoor storage area with rodent droppings and live roach also in box. Observed leaves and tree debris under boxes in storage unit.
COS CA: PIC is discarding items in box and will discard all possibly contaminated items. Facility is not to use current outdoor storage and will procure a storage unit that is designed to effectively prevent outside contamination. Single-service and single-use articles shall be stored In a clean, dry location; Where they are not exposed to splash, dust, or other contamination; and At least 6 inches (15 centimeters) above the floor.
